What this model tracks

Community health workers (CHWs) join the DPP workforce, supported by their own wellbeing and by available funding. A pipeline of teenagers gains experience and eventually becomes new CHWs. The action ideas below represent things your group could actually do — switch on any combination and see how the four outcomes below respond, compared with the goals and concerns from your workshop.
